Harnessing collective strength: Indian Society for Pediatric and Adolescent Endocrinology – Clinical Algorithms and Recommendations for Pediatric Endocrinology, Internet Protocol Detail Record, and the road ahead

The past few months have been an energizing time for Indian Society for Pediatric and Adolescent Endocrinology (ISPAE), and I am delighted to share some reflections with you.
The ISPAE conference in Nagpur in November 2025 was a vivid reminder of what brings us together: a shared commitment to improving the lives of children and adolescents with endocrine disorders, and the joy of learning, collaborating, and celebrating as a community. Nagpur offered a rich scientific feast. The meeting saw enthusiastic participation from trainees, early-career colleagues, and senior members alike. Plenary lectures, symposia, and discussions were packed, reflecting both the breadth of topics and the depth of engagement. Equally memorable was the cultural events that showcased the warmth and creativity of our hosts.
A key milestone at Nagpur was the formal launch of ISPAE working groups and Clinical Algorithms and Recommendations for Pediatric Endocrinology (CARE). These initiatives stand for our aspiration to move beyond isolated efforts and toward an integrated, equitable model of care for children, and adolescents with endocrine disorders across the country. Over time, we hope that it will help reduce regional disparities, provide practical tools for busy clinicians, and ensure that evidence-based care is not restricted to a few centers of excellence but reaches every child who needs it.
To translate these aspirations into reality, we are strengthening and expanding our working groups. These working groups will be the driving forces behind guideline development, care pathways, educational resources, registries, and advocacy efforts. In particular, we are aligning our efforts with international best practices, while remaining sensitive to the specific challenges and constraints of the Indian context. In this spirit, the ISPAE guidelines on key conditions such as diabetes, congenital hypothyroidism, hyperinsulinism, and differences in sex development (DSD) provide an essential foundation. The working groups focused on diabetes, thyroid, hypoglycemia, and DSD are already exploring ways to bridge this gap between international consensus and local implementation.
Alongside guidelines and pathways, registries will play a central role in improving future pediatric endocrine care in India. The Internet Protocol Detail Record (IPDR) registry represents a crucial step toward systematically capturing real-world data on pediatric diabetes in India. High-quality registry data will allow us to understand disease patterns, outcomes, and care gaps, and will position ISPAE members at the forefront of research. It will also help us advocate more effectively with policymakers and funding agencies when we can show robust, locally generated data. The success of the IPDR registry will depend entirely on broad participation and meticulous data entry from centers across the country, large and small.
None of these ambitions will be realized by a small core team alone. I, therefore, extend a heartfelt invitation to each of you to join working groups in areas that align with your interests and strengths. Trainees and early-career colleagues are especially encouraged to come forward; working groups are an excellent platform to learn, contribute, and build professional networks.
As we look ahead, I am optimistic. We have a motivated membership, a strong culture of collegiality, and a growing ecosystem of clinical, educational, and research initiatives. If we can harness this collective strength through well-functioning working groups and shared projects such as ISPAE-CARE and the IPDR registry, we will not only improve care for our patients today but also shape the future of pediatric endocrinology in India.
